LifeLong Links – Aging & Disability Resource Center

LifeLong Links” (LLL) is a designated Aging and Disability Resource Center (ADRC). The purpose of LLL is to expand and enhance the state’s existing information and referral resources and provide a resource to service as a "No Wrong Door" approach to long-term services and supports for persons 18 years or older with disabilities, persons 65 years old or older without regard to disability, veterans, families, and caregivers.

Aging and Disability Resource Center” – means the system that provides a single entry point/No Wrong Door access to Iowa’s long-term community services and supports. Iowa is one of 43 states that are part of the ADRC grant program, and initiative of the United States Department of Health and Human Services, jointly administered by the Administration on Aging and the Centers for Medicare and Medicaid Services. Iowa’s ADRC project is administered by the Iowa Department on Aging who contracts with local AAAs to provide ADRC Core Pillar Services.

ADRC Core Pillar Services” means:

  1. Information, Referral & Assistance

  2. Options Counseling and Assistance

  3. Independent living streamlined eligibility assistance for public programs

  4. Person-center care transitions support

  5. Quality assurance and continuous improve

Options Counseling” – means a person-centered, interactive, futures planning process whereby individuals receive decision support in deliberating their preferences strengths and values for long term support.
