Make Heritage your first call to access community resources that improve your quality of life

Aging and Disability Resource Center (ADRC): supports the long-term services and support system for older adults, adults with disabilities, caregivers, veterans and families. Its role is to provide unbiased information and counseling to those seeking long-term services and support. The ADRC is a collaborative effort among the state’s Area Agencies on Aging to provide a “No Wrong Door” entry into Iowa’s long-term services.

Information, Referral & Assistance: certified IR&A specialists listen carefully to your needs and provide you with unbiased, customized options to help you make informed decisions.

Options Counseling: helps you understand services available to address your needs and assess eligibility for entitlement programs. Options Counselors help you develop an action plan to reach your goals and follow up to ensure you receive the right level of support. Options Counseling services can be done in the home, at our offices or over the phone.

Case Management: provides care coordination in circumstances where the older adult is experiencing diminished functioning capacities and needs help connecting with in-home support to maintain health and independence.

Older Worker Employment Program: The Older Worker Employment Program (OWEP) is a partnership between Heritage AAA and Iowa Vocational Rehabilitation Services (IVRS) to empower adults with disabilities as they enter or re-enter the workforce. An emphasis is placed on the consumers’ skills and career interests to help turn passions into professions. Support is provided every step of the way, from considering careers and writing your résumé to setting up interviews and – if all goes well – getting the gig. OWEP is available to adults ages 55+ with a disability. The program requires a referral from IVRS, so call the Cedar Rapids office at 319-294-0308 or the Iowa City office at 319-354-4766 to get started.

TAILS: short for “To Aid In the Lives of Seniors,” TAILS provides emergency food, supplies and resources for consumers’ dogs, cats, fish and birds. The program is funded entirely by donations and grants, making it a real passion project for the nutrition team. Referrals are required, so contact our office to discuss your situation and needs.

Heritage also funds community resources

Adult Day Care / Adult Day Health: provides personal care for dependent adults in a supervised, protective and congregate setting.

Assisted Transportation: offers door-to-door assistance if you have physical or cognitive difficulties using regular vehicular transportation like cars, vans or buses.

Chore Services: assists if you have difficulty with activities of daily living such as heavy housework, yard work, sidewalk maintenance or home repair.

Emergency Response System: offers a fast and easy way to get help during an emergency, such as medical problems, a fall, a fire or other events that require immediate response.

Legal Assistance: provides legal advice, counseling and representation by an attorney or person acting under an attorney’s supervision.

Material Aid: helps in the form of goods or services such as food, smoke detectors, eye glasses, emergency response systems, security devices and more.

Respite Care: provides a brief period of relief or rest for caregivers by offering temporary, substitute supports for the care recipients.

Transportation: transports you when requested from one location to another via vehicle, but does not include any other activity. Sometimes called “curb-to-curb” service, it differs from Assisted Transportation in that providers may not be able to help passengers get from their door into the vehicle and vice versa.
